
Clinical standards for managing common conditions
The CareEnhance® Care Strategies module of CCMS® supports care managers with a set of clinical standards for the most commonly managed diseases and conditions. McKesson developed the content for Care Strategies using the same rigorous approach that characterizes our InterQual criteria, to deliver best-practice clinical knowledge based on literature reviews and input from practicing physicians and case managers.

Establish a comprehensive clinical framework
Each Care Strategy is made up of components that are integrated throughout the Care Manager module of CCMS. You can use any or all parts of Care Strategies once you’ve licensed the module along with the Care Manager workflow module:
- Case type specific assessments which enable gathering members' baseline condition-specific information including risk factors, diagnostic information and comorbidities.
- Stratification and clinically specific assessments so you can divide members into groups with similar characteristics such as severity level, risk factors or service needs.
- Identification of actionable health or psychosocial issues and appropriate goals that you can then follow up on.
- Tracking of key milestones and interventions; Care Strategies projects critical milestones and recommends actions based on individual progress.
- Outcomes reporting to provide insight into the effectiveness of your care management program workflow and condition-specific care plans.
- Customization utility so you can easily modify the Care Strategies content, as well as write new clinical care guidelines and protocols to address the unique needs of your case management program.
Assess general health status
The Care Strategies module also includes 14 General Assessments, such as pain management, discharge planning, cancer screening and hypertension. You can use the assessments to evaluate overall health to ensure that your member in a certain disease population is not at risk for another condition. General Assessments can also be used for members that haven’t been assigned to a specific disease population.
