Disease Monitor – a module of McKesson’s CareEnhance® Clinical Management Software (CCMS®) suite – utilizes your medical and pharmacy claims to identify and monitor more than twenty chronic diseases and other conditions early, when interventions can be most beneficial. 

Disease Monitor also allows you to easily create customized rules to identify and monitor additional diseases and conditions.

Disease Monitor can be used as a stand-alone identification and monitoring application, or to expand the disease management capabilities of the CCMS workflow module (CareEnhance Care Manager).

Benefits

  • Helps you understand the best type of intervention. Once you identify an individual member as having a chronic disease or condition, Disease Monitor applies clinical algorithms designed to detect treatment failure, complications and comorbidities, noncompliance or exacerbation of the condition. Based on these findings, Disease Monitor determines whether that member requires medical care or intervention.

  • Automates reminders or alerts you for immediate intervention. For most members, Disease Monitor will perform an automatic, low-intensity intervention, such as sending educational materials as part of a health promotion initiative or reminding a diabetic member to receive an annual eye exam.

    In cases that require more immediate intervention, Disease Monitor can alert a case manager within the Care Manager workflow module. With the help of relevant background provided by Disease Monitor, your nurses can then initiate the most appropriate and timely case management.

  • Monitors members over time. Disease Monitor continues to check on your members not referred directly to case management to help you ensure they are receiving optimal management and treatment.

  • Helps you target and evaluate your disease management program. Disease Monitor creates reports that describe disease populations, providing the information you need to understand where to focus your attention.
