Horizon Health Summary™ is the “source of truth” for patient longitudinal clinical information across your health care enterprise and episodes of care. It helps manage the complexity of care collaboration using industry standard terminologies for codification of data, facilitating medication reconciliation and patient safety.
While other vendors support one form of coding, Horizon Health Summary supports multiple clinical and financial standards from national and international organizations for multidisciplinary use such as SNOMED CT, ICD, CPT, HCPCS, LOINC, DSM and others. In addition, Horizon Health Summary provides relationships between terminologies so that data can be used for clinical and financial purposes. For example SNOMED CT clinical concepts are mapped to ICD and CPT codes. A Clinical Terminology Manager is provided for customers to maintain flexibility with standard mappings.
