Family physician brings academic, clinical experience to program BROOMFIELD, Colo., Jan. 8, 2010—Dr. Carrie Nelson has joined McKesson as the new medical director for the Your Healthcare Plus Medicaid disease management program in Illinois. In this role, Dr. Nelson works with the Illinois Department of Healthcare and Family Services (HFS) and McKesson Health Solutions staff to monitor and improve clinical program outcomes and oversee provider outreach activities. “We’re excited to have Carrie on board in Illinois. She brings nearly 20 years of medical experience to the team,” said Dr. Fredric D. Leary, senior medical director, McKesson Health Solutions. Your Healthcare Plus began helping Illinois Medicaid beneficiaries with chronic illnesses in 2006 and has steadily increased the number of beneficiaries its serves. The program currently helps 228,000 eligible participants throughout Illinois. The program employs a Care Team of registered nurses and other healthcare professionals to improve health outcomes for participants. The Care Team’s efforts to help patients manage chronic illnesses like diabetes, asthma, coronary artery disease and depression recently generated more than $104 million in net savings during the program’s second year. In addition, McKesson and HFS have extended and expanded Your Healthcare Plus through June 30, 2011. Dr. Nelson is a 17-year veteran family physician with first-hand experience working in various practice types including private, employer and academic. She has authored numerous articles and given presentations on clinical quality improvement. Most recently, Dr. Nelson served as residency director at the Rush-Copley Family Medicine Residency Program and assistant professor at Rush Medical College, where she earned her medical degree. She is second vice president of the Illinois Academy of Family Physicians, chairperson of the Coalition for Quality and Patient Safety of Chicagoland and vice-president of the Institute of Medicine–Chicago.
