New Program from Illinois Department of Healthcare and Family Services, McKesson OAK PARK, Ill., Sept. 25, 2006—Fredric “Rick” Leary, M.D., has been named Medical Director for Your Healthcare Plus, the new chronic illness management program for adults and children administered by the Illinois Department of Healthcare and Family Services and McKesson Health Solutions. Dr. Leary is the Past President and Chair of the Board of the Illinois Academy of Family Physicians and a fellow of the American Academy of Family Physicians (AAFP). In these capacities he has represented the interests of primary care on numerous local, state and national committees. Your Healthcare Plus features comprehensive medical management designed to help adult Illinois Medicaid beneficiaries who have chronic illnesses. Children or adults with persistent asthma can participate, as well. The program began July 1, 2006. “Rick will work closely with Illinois physicians and provider groups to assure that Your Healthcare Plus creates and maintains a positive relationship between the program and providers,” said Maureen Mangotich, M.D., chief medical officer and senior medical director Clinical Outcomes at McKesson Health Solutions. Technology makes the program easy for providers to use by making educational materials, provider reports and patient profiles available online. Your Healthcare Plus promotes and encourages identification of a medical home – a primary care provider for each beneficiary – for all Medicaid beneficiaries and children in the state healthcare programs. Your Healthcare Plus assists program participants in locating a medical home if they don’t have one. Dr. Leary, who has 15 years experience as a family physician in Illinois, has practiced in a variety of settings including private practice, a rural health clinic and most recently as a family medicine residency instructor. His professional interests and practice designs focus on quality improvement and patient safety. He has worked with the American Medical Association Physician Consortium, the American Board of Family Medicine in developing their performance in practice modules, the AAFP in creating evidence-based performance measures and the Quality Improvement Organization in Illinois.
