Regence expands access to member-focused McKesson care management programs McKesson and Regence BlueCross BlueShield today announced a partnership to provide additional and improved chronic illness management services to nearly 140,000 Regence members who now have access to McKesson’s care management service, Personal Care Manager. Personal Care Manager utilizes technology, nurse care managers and dedicated provider outreach to engage members and ensure they learn new ways to manage chronic conditions, stay healthy, and avoid unnecessary hospitalizations and other adverse health events. Using detailed computerized updates on the progress of each member, nurse care managers individually tailor treatments in real-time. “We believe Regence members will experience better health outcomes as a result of this partnership with McKesson,” said Regence Chief Medical Officer Ralph Prows. “The ability to track the progress of each member, and to be more coordinated with providers, gives us real confidence that this program will work for our members.” This multi-faceted approach allows the program to impact more members than comparable services and engage them with appropriate health resources. Personal Care Manager’s targeted approach and ability to connect with members at the right time drives better clinical outcomes and more cost savings for health plans. “Personal Care Manager is McKesson’s multi-channel, data-driven member and provider engagement platform that ensures health plan members control chronic illnesses. Provider support is delivered through McKesson’s provider portal and regionally-based nurse care managers who assist medical practices that support Regence members. The portal ensures providers can track the progress of Regence members at any time, supporting a more coordinated care approach among the health plans, providers and care management program. The nurse care managers ensure alignment between the program’s care plan and the provider’s treatment plan. Regence also has unprecedented access to detailed program data, which enables real-time updates on the performance of the program.
