ACCESS Plus program serves low income children and families BROOMFIELD, Colo., Nov. 24, 2008—McKesson Health Solutions, a business unit of McKesson Corporation, and the Pennsylvania Department of Public Welfare (DPW) today received the 2008 Outstanding Provider Engagement Initiative Award from DMAA: The Care Continuum Alliance. The organizations accepted the award at DMAA’s national convention in Florida. “ACCESS Plus is one of those unique programs that highlights the best of what care management services can offer when a holistic approach is taken among the payor, provider and beneficiary. By developing relationships with providers, including pay for participation, we’ve been able to align the chronic illness management program with the medical home, which benefits the beneficiary and the physician’s office. This new payor/provider collaboration has produced a significant ROI for the program,” said Emad Rizk, M.D., president, McKesson Health Solutions. McKesson has provided disease management services to DPW since March 1, 2005. The program, called ACCESS Plus, uniquely integrates an enhanced primary care practitioner network with disease management and serves approximately 295,000 Medicaid beneficiaries in the Commonwealth’s 42 rural counties. “Our program supports quality improvement at the primary care practice level and then helps patients access those practices for care. Our clinical, financial and satisfaction results show the benefits of closing the payer-physician-patient triangle,” said Lonnie Fuller, M.D., medical director, ACCESS Plus/McKesson Health Solutions. ACCESS Plus delivers healthcare services to low-income children and families. The program has improved quality of care and helped participants better manage chronic illnesses such as diabetes, heart problems and asthma. ACCESS Plus is administered by McKesson Health Solutions in partnership with Automated Health Systems. “McKesson Health Solutions has seen outstanding results in its state programs, and its collaboration with Pennsylvania on provider incentives is yet another example,” DMAA President and CEO Tracey Moorhead said. “McKesson’s work supports providers in chronic condition care and demonstrates the value of population health.” In another program achievement, earlier this year ACCESS Plus was named one of 15 finalists for the 2008 Innovations in American Government Awards.
