BROOMFIELD, Colo.--(BUSINESS WIRE)--The New Hampshire Department of Health and Human Services (DHHS) and McKesson Health Solutions, a unit of McKesson Corporation, today announced that the state’s disease management program has received the 2007 “Outstanding Government Program, Disease Management Leadership Award” from the Disease Management Association of America (DMAA). McKesson provides the disease management services to New Hampshire. “New Hampshire’s successful strategy is to focus on the individual, not the disease,” said Doris H. Lotz, M.D., MPH, medical director for New Hampshire’s Medicaid program. “We look at our recipients’ understanding of their disease, their readiness for self improvement and, very important for a Medicaid population, the psychosocial barriers to health improvement that they encounter. Our disease management program helps us take significant steps toward accomplishing the health and wellness goals of recipients and the state.” DHHS began its statewide disease management program in 2005 and includes 24/7 nurse advice services for Medicaid clients, as well. McKesson provides services to Medicaid clients who have asthma, diabetes, heart failure, coronary artery disease, COPD (chronic obstructive pulmonary disease), end stage renal disease and chronic kidney disease. “DHHS is to be commended for its continued commitment to improve the self-management skills of Medicaid clients. This award is a reflection of the progress of the program, which serves the state and Medicaid clients by improving clinical outcomes and delivering ROI,” said Emad Rizk, M.D., president, McKesson Health Solutions. According to the DMAA Program Guide: “This program produced positive results across the board, including return on investment, reduction of emergency department admissions and high levels of beneficiary satisfaction. Achieving substantial results is a challenge because of the geographical dispersion of residents — New Hampshire has fewer people living in metropolitan areas (60 percent), compared with the national average of 80 percent.
