Pennsylvania Department of Public Welfare Renews Disease Management Contract with McKesson Health Solutions

BROOMFIELD, Colo.--(BUSINESS WIRE)--McKesson Health Solutions, a business unit of McKesson Corporation, and the Pennsylvania Department of Public Welfare have announced that the State has renewed its disease management and primary care case management programs. The renewal began Oct. 1, 2007 and lasts through June 30, 2009. The programs began on March 1, 2005.

The programs will continue to provide services to about 290,000 eligible children and adults who live in the Commonwealths 42 mostly rural counties. Nursing staff helps those with asthma, diabetes, COPD, coronary artery disease and heart failure.

With DPWs support, were making several significant changes that will help improve program outcomes. McKesson will utilize the community-based model that weve successfully employed with other Medicaid programs, more sophisticated predictive modeling and increased interaction with high-volume practices, said Emad Rizk, M.D., president, McKesson Health Solutions.

The current pay-for-performance (P4P) program will be expanded to include 27 new beneficiary-related measures to encourage provider participation. New P4P measures include:

  • Retinal eye exam by eye care professional
  • HbA1c testing
  • Obtaining an LDL test

Additional community-based interventions include McKesson nurses who will travel to the homes of beneficiaries to administer the disease management program.
