Baptist Health, a ten acute care hospital system in Kentucky, faced declining Medicare payments and potential penalties if it failed to control costly patient readmissions. By employing McKesson Performance Analytics™ to model and identity potential at-risk patients, Baptist Health was able to develop a system of interventions that improve patient outcomes while reducing cost. Using real-time tracking for patients with a high potential for developing conditions that would require Emergency Department (ED) or hospital care, the organization reduced readmissions 4%. The health system was also able to model and track the performance of both hospital services and outpatient centers so that managers can evaluate performance and drive greater efficiency and savings within the system.
